Dispensing
practices are now totally, financially responsible for collecting
dispensing charges from their patients. Get it wrong and each
chargeable item will cost the practice £6. So it is essential for
doctors, dispensers and patients to fully understand the new FP10
NC. Patients, pharmacists and doctors may be investigated for
fraudulent declarations on the form.

NO........ fill in Parts 1 & 3 YES........... fill in Parts 2 & 3 _____________________________________________________________ If the No box is filled then it is vital that the evidence for it is shown to your dispensers and clearly indicated on the form with a cross in Part 1. That includes the extra boxes on the right of Part 1 referring to the details for the various benefits. Part 3 should then be carefully completed by the patient or the patients representative. As should be the printed name and address and, most vital of all, a signature. Without a signature the form will be rejected and the practice will bear the cost as if dispensing charges should have been paid. The printed name is NOT sufficient. There will be no exceptions and no opportunity to represent the script. £6 per item will have been needlessly lost. Remember, prescription charges are deducted at source by the PPA nowadays. If the Yes box is filled then the amount collected from the patient should be entered in Part 2 and Part 3 should be completed. "Evidence not seen" box. Do ensure that, if appropriate, this box is ticked because it is those patients whom the Fraud Squad will check first. Especially those who repeatedly fail to show their exemption forms of whatever sort. Common errors leading to practice liability 1. No signature in Part 3, just a printed name 2. The No box completed with no reason in Part 1 3. Part 1 "Benefit" details not completed. They will be checked and patients will be prosecuted if fraud is detected. An absence of details will leave the practice responsible for the charges. 4. Rubber stamps being used in Part 3 with the practice address. Exceptions sometimes allowed Dispensing practices may sign for drugs left at remote "drop-off" points where it is not good policy to leave the FP10 for fear of it being mislaid and not being sent to the PPA for payment. A small proportion of patients cannot write and an "X" may be allowed for the signature in Part 3. Comment It is not the practice's responsibility to check the truth of patient statements in Part 1 other than to see the appropriate form. The patient's signature places the responsibility on them and they will be prosecuted if found out. They may also be checked if they do not provide evidence. So far there has been a reduction in losses of around 40% bringing the amount lost to less than £100Million. During 1998-99 19% of prescriptions were under fraud investigation. New equipment has significantly increased that figure in 1998-99. Fraud is being taken seriously and is never, ever worth the risk. May, 2000
